You, the undersigned, hereby declare that:

You have no medical issues! Your health condition is normal, and you do not suffer from any limitations preventing you from participating in strenuous physical activity.
You commit to updating any changes in your health condition.

If you have a medical issue/are taking medication:

You must provide a medical certificate from a specialist physician approving your fitness for the specific sports activity at the community center.
This certification should be sent to the secretariat via WhatsApp: 050-2025909.

If you are pregnant:

You are aware that due to the nature of the activity, you must consult with a physician, obtain specific approval, and present it to the secretariat (text the relevant approval to: 050-2025909).

Remember that you must renew this declaration before each activity and in any case of a change in your health condition.

Before registering, carefully read the waiver and release of liability document - risk-taking by choice

A participant requesting to register for sports activities at the Tirat Carmel community center (hereinafter: "the community center") must sign this waiver.

In the interest of transparency, a document of this type affects legal rights, so you must read and understand the document before signing:
Be aware that according to amendments 10 and 15 to the Sports Law, 1988, the community center is not obligated to insure athletes in popular activities, so it is important that each participant obtains personal accident insurance that provides full coverage for the nature of the activity to avoid exposure in case of injury or accident.

Below are the waiver details you are required to approve before registering:

1. Community Center Activities:
   I request to participate in the activities, training, events, and competitions of the community center (hereinafter: "the activity"), organized by the Tirat Carmel community center and/or any of its representatives.
   I declare that I am aware it involves risks which I take upon myself, and agree to release the community center from liability for any physical, mental, or property damage that may be caused to me.

2. The Risks Involved:
   I understand that the sporting activity involves inherent risks (injuries, falls, physical exertion, etc.).
   The activity is characterized by unique rules and laws related to ball games and changing physical activities.

3. Risk-Taking by Choice:
   I declare that I am aware of the risks and have decided to participate in the activity of my own free will.
   I declare that I will not have any claims or demands against the community center for damage caused to me during the activity, and that the responsibility is mine, without affecting my right to claim against my personal insurance company.

4. Health Declaration:
   I declare that simultaneously with signing this, I have signed the "Health Declaration Form" and that all details therein are accurate.
   I declare that I hold a valid medical certificate per the Sports Law if required.

5. Personal Accident Insurance:
   I am aware that I am responsible for purchasing personal accident insurance that covers the activity.
   If I do not purchase such insurance, I will not make any claims or demands against the community center.

6. Release of Liability and Indemnification:
   I release the community center from any liability for damage that may occur to me and commit to indemnify the community center for any lawsuit filed by me or on my behalf contrary to this waiver.

7. Commitment to Follow Instructions:
   I commit to follow the coaches' instructions, the community center's regulations, and the ethical guidelines of the activity.

Remember that two approvals are required before purchasing a ticket for the desired open network date.
